 Our clients understand and acknowledge that each and every transition situation is different and unique. We always maintain your privacy, sharing personal information on a “need-to know” basis only. During our client inquiry process, we will ask you for pertinent health information, preferences, and available financial resources. Other personal information that has been provided by the client or the client’s family to the agent(s) at 564-224-8847 may be shared with the communities or Adult Family Homes to be toured. We collect only the necessary information required to make our professional referrals.   

Medical professionals may be involved: To best determine the medical health condition, medication management, and appropriate care level prior to admission to a residency, the physician may be required to be involved for the best possible outcome. CCSR offer assessments, paid for by the Client, through qualified registered nurses or the Provider may provide assessment services. However, CCSR does not offer medical advice, does not assess medical needs and does not offer care plans. Once the Provider reviews the assessment, the Provider decides whether to accept the Client.
